
Discharging Patients from Specialist Palliative Care
A patient may be discharged from the hospital palliative care team if/when:
- The patient and/or family/whanau needs have been met such that the primary clinical team can continue management.
- It is felt that the patient’s needs would be best met by another service.
- The patient declines further treatment/support.
- It is mutually agreed that further input would be of no benefit to the patient.
- The patient is deceased.
Re-referrals to the hospital palliative care team can be made as often as needed.
